We require a copy of your referral prior to making your appointment. Typically this will be sent directly to us by your referring Doctor and we will then contact you directly. You will need to complete a Coastal Neurophysiology Patient Questionnaire prior to your appointment. This can be done online, as a hardcopy or you can arrive 15 minutes early for your appointment and complete it at the clinic.
